ASSUMPTION OF RISK WAIVER I hereby agree, the participation of the child named above in any sport activity, fitness, strength, training, games, and/or activities organized, operated or conducted by V3 Basketball Association alone or with another organization (collectively the "Activity"): THAT I AM assuming all health risks and adverse health-related consequences, including without limitation COVID-19, to me and the child named above caused by or arising from engaging in any V3 Basketball Association Programs (the “Assumed Risks”); TO RELEASE V3 Basketball Association and their respective directors, officers, employees, agents, volunteers, contractors, representatives, successors or assigns (collectively the “Releasees”) from any liability for any loss, damage, injury or expense, all manner of actions, causes of action, debts, accounts, bonds, contracts, claims, and demands for or by reason of any injury to person or property, including injury resulting in the death of the Participant, which has been or may be sustained as (collectively “Loss”) that I may suffer arising out of or connected with the Assumed Risks and/or the participation in and transportation to or from any V3 Basketball Association Program, due to any cause, including negligence; TO WAIVE ANY CLAIM that I have or may have against any or all of the Releasees regarding any matter, including without limitation, any claim arising out of any V3 Basketball Association Program or Activity; TO INDEMNIFY THE RELEASEES from any and all claims, actions, or Loss resulting in any way from my participation or participation of the child named above in any V3 Basketball Association Program; THAT I AM (or the child named above is) physically fit to participate in any V3 Basketball Association Programs; THAT I AM a legal guardian or custodial parent of the child named above; THAT V3 Basketball Association is authorized to record personal information, images, and audio recordings in any medium of the child named above or me at or in relation to any V3 Basketball Association Program or Activity for publicity and promotional purposes, and I hereby consent to the collection, use, and disclosure of such personal information about me and/or the child named above; THIS DOCUMENT SHALL bind my heirs, executors, administrators, assigns, and personal representatives and will have effect throughout my registration in V3 Basketball Association and, to the extent reasonably necessary to give it effect, thereafter. Terms and Conditions I hereby affirm that the information provided in this registration form is accurate, complete, and honest. I am committed to complying with all the rules and regulations of the V3 Basketball Association. I am aware that all application fees, deposits and tuition paid for the REP season are non-refundable unless stated otherwise. I am aware that if any information in this registration form is found to be false, V3 reserves the right to withdraw the offer and dismiss the athlete without a refund of any fees paid. I understand that the commitment to V3 Basketball Association, is for one full season from September 2024 to April 2025 and our season fee includes March Break Camp and 6 weeks of Summer Camp. I agree to pay the tuition fee for the season as outlined on my invoice and if paying by instalments, all instalments are to be paid on time or they can be assessed a 2% late fee. V3 Basketball Association evaluates all applicants without any consideration of race, color, creed, sex, gender, sexual preference or orientation, disability, or national origin. By submitting payment below, I understand that I am agreeing to these terms and conditions. This holds the same validity as a physical handwritten signature on a paper form according to V3 Basketball Association's policies. *
